A Partnership Between Yellow Springs Police Department (YSPD) and Miami Township Fire‑Rescue (MTFR)
The Safe at Home Call List is a collaborative wellness-check program jointly operated by the Yellow Springs Police Department’s Community Outreach Specialists and Miami Township Fire‑Rescue’s Community Paramedic. Our shared goal is to support at‑risk residents by making brief, scheduled phone calls, once weekly and before severe weather events, to ensure safety, identify emerging needs, and connect individuals with appropriate assistance.
By combining the strengths of law enforcement, fire/EMS, and community outreach, we help residents maintain safety, independence, and peace of mind in their own homes.
Our Mission
Through this joint initiative, YSPD and MTFR are dedicated to:
- Providing routine wellness check‑ins for at‑risk members of the community
- Conducting pre‑weather calls before extreme heat, cold, storms, or outage risks
- Identifying unmet medical, social, or safety needs early
- Coordinating follow‑up assistance through family, neighbors, social services, or emergency responders
- Supporting safe and independent living for vulnerable residents
Our philosophy is simple: caring, connection, and prevention through partnership.
🔍 How the Program Works
Who We Call
- Older adults living alone
- Individuals with chronic health issues
- Residents who rely on electricity-dependent medical equipment
- People with limited mobility or limited support networks
- Anyone referred by a family member, friend, healthcare provider, or community agency
Call Frequency
- Weekly check‑ins at a scheduled day/time
- Additional calls before dangerous weather, such as extreme heat/cold, winter storms, or high‑wind events
What Happens During a Call
- A quick, friendly wellness check (2–5 minutes)
- Simple questions to confirm safety, medication access, utilities, food/water, and weather readiness
- Guidance or resource support if needed
- With permission, contact to a designated friend/family member if concerns arise

Enrollment Information We’ll Request
- Name, address, preferred phone number
- Best call time(s)
- Emergency contacts
- Relevant medical or mobility considerations
- Any electricity‑dependent equipment (oxygen, power chair, ventilator, CPAP)
- Consent to share information with designated contacts
If We Cannot Reach You
- A second attempted call
- Contacting your designated emergency contact(s)
- If concerns remain and safety is uncertain, YSPD may conduct a welfare check in collaboration with MTFR if needed
🌦️ Inclement Weather Call Triggers
We will make an effort to call before:
- Extreme heat or cold
- Winter storms (snow, ice, wind chill)
- Severe thunderstorms or high‑wind advisories
- Events likely to cause power outages
Weather check‑ins focus on confirming:
- Access to medications
- Heat/cooling capability
- Backup power or safe place to go
- Food and water availability
🔐 Privacy & Consent
- Information is kept secure and shared only with YSPD, MTFR, and your approved contacts
- No information is used for non‑safety or non‑emergency purposes
- You may withdraw from the program at any time
